Questionnaires on Embodia - Creating questionnaires from scratch
To create a questionnaire from scratch go to HEP > Questionnaires > Add questionnaire. 
Give your new questionnaire a Name, choose the questionnaire Type, select a Scoring strategy, and make your questionnaire Active by checking the button to the left of the word active.

Click Submit to create your questionnaire. You will now see an empty questionnaire and can add items to it.

Click New item (#1 in the image above) to add items to your questionnaire. You can choose from the following types:
- Instruction: This item type is not fillable by the patient. Use it to provide context or instructions that help patients understand how to complete the questionnaire.
- Question: This item type is fillable by the patient. Patients will provide their answers directly. Learn more about the available question types in our help article, Question types on Embodia.
- Connect a patient property: Similar to questions, this option links a patient property to your questionnaire. When a patient answers this question, their response automatically updates the associated patient property on their profile. (Available for Tier 3 memberships only.) Learn more in Using patient properties.
Should I Connect a Patient Property or Add a Question?
Use Connect a patient property if the information changes infrequently and should be stored as part of the patient’s profile. For example, date of birth, address, or phone number are best added this way — these responses will automatically update the patient’s profile.
Use New Question for all other information that’s specific to a consultation, visit, or a clinical case. For instance, if you want to ask, “What is the reason for booking this consult?”, this should be added as a New question rather than a patient property.
